The government has announced a new initiative called Diagnosis Connect, aiming to give immediate support to people diagnosed with long-term conditions. From 2026, patients will automatically be referred to specialist charities as soon as they receive a diagnosis, starting in GP surgeries. This quick connection to trusted organisations will provide tailored advice, emotional support, and practical guidance right from day one-complementing, not replacing, their NHS care.
Key areas targeted in the initial rollout include diabetes, lung conditions, and mental health, with more conditions added over time. The system will start with around 250,000 people in its first two years but is designed to expand to hundreds of thousands in the future.
This partnership between the NHS and major health charities aims to reduce hospital visits, improve patients’ confidence in managing their conditions, and address health inequalities. Patients will be notified and connected with support groups via text or the NHS App, ensuring help is accessible from the moment of diagnosis.
